Diagnostic performance of coronary CTA and CT-FFR for the detection of coronary artery disease in routine TAVI work-up

Abstract Objectives To assess the diagnostic performance of CT-FFR for diagnosis CAD in work-up TAVI. Background Work-up transcatheter aortic valve implantation (TAVI) currently utilizes computed tomography (CT) to evaluate annulus diameter and peripheral vascular access, plus invasive coronary angiography (ICA) significant artery disease (CAD). ICA might partially be redundant with use Coronary CT Angiography (CCTA). Prior studies found improvement accuracy CCTA by derived fractional flow reserve (CT-FFR). Methods Consecutive patients severe symptomatic stenosis who underwent TAVI between 2015–2019 were included this cross-sectional study. All within 3 months both assessed using as reference. Results Seventy-six 338 analysis had ≥1 at ICA. The sensitivity, specificity, positive predictive value, negative per-patient 76.9%, 64.5%, 34.0%, 92.1% 66.9% 84.6%, 88.3%, 63.2%, 96.0% 87.6% CT-FFR. area under receiver-operating characteristic-curve significantly differ (0.84 versus 0.90 p=0.02). A guided approach could avoid 57.1% 43.6% CCTA. Conclusions improves without additional testing increases proportion whom have been safely avoided It has potential integrated current clinical diagnosing stable requiring treatment.
